left an employment (or opted out) with deferred benefits and did not have a continuous break of more than 5 years in active membership of any public service pension scheme, or
left an employment (or opted out) with deferred benefits or ceased a concurrent job with a continuous break of more than 5 years in active membership of any public service pension scheme, or
left an employment (or opted out) before 31 March 2014 and did not have a continuous break of more than 5 years in active membership of any public service pension scheme and rejoined the LGPS after 31 March 2014, or
left an employment (or opted out) before 31 March 2014 and had a continuous break of more than 5 years in active membership of any public service pension scheme and rejoined the LGPS after 31 March 2014
